Please include your name, the e-mail you used when joining the club and your username and a current e-mail if it is different than the e-mail you used when joining.

If you don't have all of these, give us what you have ... but be sure to give us an email where we can contact you in case we have trouble finding your record.

Once you tell us to cancel your membership, you will not be billed for any additional membership charges.
